Application:  Be sure to include a recent photograph of yourself with your application*.  Be certain to include the first date you will available for work and the last day you will be available for work on your application (this means the dates you can punch in on the time-clock ready to go to work).  Your contract will be based upon these dates.

Put down current telephone numbers of all past employers and references you list.  If you do not include the telephone numbers or if we are unable to contact these people, we may not be able to hire you even though we would like to.  If you have an e-mail address, include this contact as well.  E-mail, fax, or mail your application back to us immediately.  *If you are e-mailing or faxing, please send a recent photograph under separate cover.  We will begin calling references as soon as we receive your application.
